2020 School-based Influenza Vaccination Program

The Department of Health (DOH), in collaboration with the Department of Education, will again be conducting the Stop Flu at School program, a statewide school-located influenza vaccination program. Due to funding reductions and limited resources, the program has been scaled back and will no longer be offered in private schools. Free flu shots will be offered to all elementary and middle public schools on the neighbor islands and selected public schools on Oahu. Parents must complete a consent form for their child to participate. Flu Shots will not be offered to faculty and staff.

Note: The SFAS online school registration system does not accept symbols or special characters, including diacritical marks, such as kahakō and ‘okina, and will not allow you to submit or complete your entry. Please do not use any symbols or special characters, including diacritical marks, when inputting registration information.

To participate as a school:
1. Identify a staff member to serve as the SCHOOL LIAISON, responsible for:

  • Working with DOH staff and DOH agents to schedule and prepare for the clinic and consent form review dates (i.e., prescreening);
  • Notifying school personnel and parents of the clinic date and time;
  • Distributing and collecting consent forms;
  • Coordinating parent and school volunteers;
  • Escorting consenting students to and from the vaccination clinic;
  • Ensuring consistent and smooth flow on clinic day (e.g., determining the class schedule, escorting students to and from clinic when necessary, providing information to the Clinic Manager during operations, etc.)

2. Indicate the dates between October 12, 2020 and November 20, 2020 when a vaccination clinic CAN be held.

  • Clinics will not be held on State/Federal holidays. Oahu clinics will not be scheduled on Mondays, nor the day after State/Federal holidays.
  • Once your clinic date and time are set, it will not be possible to reschedule the clinic. IMPORTANT: Please keep your scheduled clinic date and time in mind when arranging field trips, assemblies, etc.

3. Reserve a LARGE space (e.g. gymnasium, auditorium, cafeteria) for a 4-hour vaccination clinic (on average), including a designated waiting area for clinic staff to monitor students for 15 minutes after being vaccinated.

5. Mark your calendar with these important dates:

Registration deadline Friday, April 17, 2020
Consent forms distribution June 29 - July 10, 2020
Schools may choose to distribute SFAS consent forms in student “back to school” packets, or distribute in another way that is more suitable for schools’ needs
Consent form deadline Thursday, August 20, 2020
Consent form review August 24, 2020 – September 4, 2020
Clinic date To be scheduled October 12, 2020 – November 20, 2020
